The LDA APRN/Primary Care will join the ROAD MAP (Rural Health and Older ADult Interprofessional Mobile HeAlth Program) team.
ROAD MAP is an initiative led by the University of Tennessee at Chattanooga School of Nursing that assists older adults living in rural Tennessee counties who are at a higher risk for developing chronic diseases, cognitive disorders, and poor health outcomes due to a lack of access to health care, continuity of treatment and resources. This initiative aims to mitigate those risks by delivering health care and social services professionals to rural communities.
The position will be a full-time health care provider on, and clinical manager for, the Mobile Health Unit. As a member of an interprofessional team, the APRN will provide and supervise health care for individuals and caregivers in the community who present for evaluation and treatment or are participating in associated community health programs. The provider will be the subject matter expert caring for vulnerable individuals in the community including planning, prevention, detection, and management of chronic diseases. In addition, the APRN will oversee and manage clinical and administrative tasks related to the Mobile Health Unit.
Duties and Responsibilities:
Provides assessment, diagnosis, treatment, education, referral, and follow-up for mobile health unit patients, including telehealth visits; manages multiple chronic illnesses, recommends screenings and tests based on patients' risk profile; manages health care within protocols mutually agreed upon by the supervising physician and Mobile Health team members; identifies abnormal findings early and initiates appropriate intervention; and assesses and provides support for the cultural, psychosocial, language and physically challenged needs of the older adult and their care partners.
Serves as the primary contact for mobile health unit; provides supervision of all staff, faculty, and students during Mobile Health service hours; ensures clinic is operational and functions within the established policies, procedures and protocols; and maintains and abides by a collaborative practice agreement with the supervising physician.
Works with the Interprofessional Education and Practice Coordinator to schedule and train those who serve on the Mobile Health Unit; serves as a liaison with partner sites and community stakeholders; and contributes to program design, implementation, and evaluation through feedback and meeting attendance.

This position is a Limited Duration Appointment (LDA).
A Limited Duration Appointment (LDA) is a form of regular employment for a specific project or for a limited period. An LDA may be established for up to one year with an option to renew the appointment annually for a maximum of three years. Please note that this position may be converted to a permanent position prior to or at the conclusion of three years, funding dependent.

Minimum qualifications: Master's degree and license to practice as a Registered Nurse; certification in the area of training and practice from a nationally recognized certifying body required; licensed (or eligible for license) to practice as a Nurse Practitioner in Tennessee; eligible for prescriptive authority application through the Board of Nursing or Medical Board in TN; BLS for Healthcare Provider from AHA required with expiration date greater than 90 days from date of beginning practice.
Preferred Qualifications: Doctorate of Nursing Practice; one year experience providing primary care for older adults in an outpatient or community health setting; at least one year employed as a licensed nurse practitioner; one year experience practicing on a Mobile Health Unit; bilingual in Spanish and English.
