Job Summary:

This is a collaborative position between the Children's Hospital Immunodeficiency Program (CHIP) and the University of Colorado HIV/AIDS Clinical Program. The position is based in the Infectious Disease Group Practice (IDGP) at the University of Colorado Hospital but is also expected to attend and participate in youth and/or pediatric clinics at CHIP.

The IDGP is a busy, fast-paced adult clinic with a large patient population experiencing financial, mental health, substance, social and health related issues. CHIP serves children, youth, pregnant women, and families infected, affected, or at-risk for HIV infection.

This position primarily involves the medical case management of a caseload of people living with HIV. Part of the patient support team, the role will provide medical case management and supportive services to patients living with HIV.

Key Responsibilities:
  • Serves as liaison for collaboration of care services between IDGP and CHIP.
  • Provides medical case management to a population of female and male patients who are or have recently transitioned to IDGP from CHIP; as well as women in care at the IDGP.
  • Assists patients with transition between providers from CHIP to IDGP.
  • Tracks and documents transitioning patients in order to provide support and linkage to care between CHIP and IDGP. Continues to assess needs and address any barriers to care.
  • Conducts initial and ongoing psychosocial assessments with patients to assess needs (e.g. mental health, substance use, financial, and housing).
  • Advises and provides assistance to patients in obtaining social, financial, housing, insurance, and other needed services.
  • Works as part of multidisciplinary teams at IDGP and CHIP, including care coordination activities and attending team meetings.

Work Location:

Hybrid - this role is eligible for a hybrid schedule of 3 days per week on campus and as needed for in-person meetings.

    Qualifications:

    Minimum Qualifications:
    • Bachelor's degree in any field.

    A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.

    Applicants must meet minimum qualifications at the time of hire.

    Preferred Qualifications:
    • Bachelor's degree in human services field.
    • One (1) year of case management or related experience.
    • Experience in HIV care.

    Anticipated Pay Range:

    The starting salary range (or hiring range) for this position has been established as $60,000 - $65,000.
